What We Offer

  • $41.66-$51.03/hour DOE with the ability to go higher for highly experienced candidates

As a Registered Nurse, you will have the opportunity to:

  • Provide comprehensive care during patient visits, discuss chief complaints, obtain vital signs, complete health history, and document health information in the EMR system
  • Conduct phone triage to assess patient needs and coordinate follow-up care
  • Assist with diagnostic procedures and treatments, including IV starts when ordered, while educating patients on procedures and medications
  • Collaborate with the healthcare team to develop and implement patient care plans, coordinate referrals, and provide education to patients on medications and potential side effects

Qualifications:

  • Associate’s Degree or higher in Nursing
  • OR State RN license; Dual licensure in WA & OR is required within 120 days of hire
  • If an RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply:Must submit proof of license in state of residence.Must submit proof of completion for the required suicide prevention training during 1st week of onboarding.If relocating to Washington State, must obtain WA RN license within 6 months of hire.Must update the primary state of residence within 60 days (applies to anyone relocating to Washington State).Military Nurses are not required to obtain a Nurse Licensure Compact (NLC)
  • BCLS certification within 90 days of hire; ACLS & PALS are preferred

This position provides clinical and administrative oversight to RNs, mental health workers, case managers, and service care coordinators, who provide maternity and infant case management to eligible clients. This role works in close coordination with the clinic administrator, program directors, and community partners. The ideal candidate must be able to collaborate and engage with service providers to increase referrals and services.

Who We Are Looking For:

  • A Registered Nurse who is passionate about community health

  • A compassionate, caring, person who has the desire to work with maternal and pediatric clients throughout the pregnancy and maternity cycle

  • A clear communicator who can build strong relationships with patients, CHS staff, and clinic staff

  • A skilled nurse proficient in NANDA diagnosis and therapeutic communication

Position Highlights:

  • $95K-$125K DOE with the ability to go higher for highly experienced candidates

  • $9,500 Hiring Bonus Structure:

    • At Hire: $2,850.00

    • At 180 Days (6 months): $3,800.00

    • At 12 months: $2,850.00

  • Bonus will be pro-rated based on Full Time Equivalency (FTE)

  • An additional 4% differential for your bilingual skills

  • Relocation assistance may be available for candidates relocating over 50 miles

What You’ll Do:

  • Coach, mentor, and train staff. Manage performance and provide feedback.

  • Support staff growth. Assess educational needs and encourage job-related training.

  • Recruit and select staff. Provide orientation on job requirements, policies, and tools.

  • Review maternal and pediatric referrals from Case Management and clinic staff.

  • Administer screening and assessment tools.

  • Develop client Care Plans based on health history and screening data.

  • Maintain therapeutic client relationships, set goals, monitor progress, and revise Care Plans.

  • Perform prenatal, postpartum, and newborn care interventions and education, including home visits.

  • Participate in interdisciplinary team case reviews.

  • Maintain required documentation.

  • Mentor nursing students, community health workers, and new Public Health Nursing staff.

  • Review staff charts for policy compliance.

  • Ensure site productivity and work with staff on improvements.

Qualifications:

  • Bachelor’s Degree in Nursing with coursework in public health

  • Two years’ experience in providing maternity and newborn physical exams. This includes at least one years’ supervisory experience.

  • Current WA state RN license

  • CPR certification within 45 days of hire

  • Bilingual (English/Spanish) skills are preferred

  • Valid Driver’s License and proof of automobile liability insurance coverage is required

What You’ll Do:

  • Patient Triage:Assess the urgency of patient needs, gather pertinent information to ensure appropriate responses, and direct care as necessary.
  • Patient Inquiries:Respond to patient questions regarding medications, treatments, and general healthcare information.
  • Care Coordination: Guide patients to the right resources, send messages to providers, and advise on next steps. Work with the healthcare team to create and implement care plans, coordinate referrals, and educate patients about medications and side effects.
  • Efficient Communication:Collaborate with our local team of nurses to ensure consistency, quality, and warmth in every patient interaction.
  • First-Call Resolution:Aim to resolve patient concerns during the initial call to foster trust and patient satisfaction.
  • Comprehensive Care: During patient calls, discuss main concerns, take health information, and document information in the electronic medical record (EMR) system.

What We’re Looking For:

  • Licensed Registered Nurse:An active Washington RN license is required.

What You’ll Do:

  • Obtains complete history and conducts chart review, ensuring inclusion of all reports needed.
  • Performs baseline physical assessment and expands assessment based on initial findings.
  • Collaborates with health care Providers in developing and implementing plan of care.

Qualifications:

What You’ll Do:

  • Coaches, mentors and trains direct report staff. Assesses competencies of staff, provides continuous performance management and conducts employee performance evaluations.   • Collaborates with Nursing and Medical Assistant staff to create and implement ideas for education including clinical performance and effective communication.
  • Participates in the recruitment and selection of staff. Provides orientation to outline job requirements and expectations, policies and procedures, and proper use of tools and equipment.
  • Develops and implements annual department work plan forecasting equipment needs and capacity requirements.
  • Delegates, assigns and coordinates work, ensuring an efficient work flow that achieves the departmental mission and objectives.
  • Monitors staff compliance with all policies, procedures and standards of Yakima Valley Farm Workers Clinic (YVFWC). Ensures clinic compliance with Health Insurance Portability and Accountability Act (HIPPA), The Joint Commission (TJC), and Occupational Safety & Health Administration (OSHA) standards of patient and employee safety.
  • Collaborates with management in the clinic as well as other clinics to plan patient care delivery systems to effectively address patient needs and ensure quality patient care.
  • Leads the Nursing and Medical Assistant staff in the daily implementation of patient care plans and workflows that meet state and federal requirements and/or metrics.
  • Reviews processes related to daily implementation of patient care and assignments. Evaluates adherence to process and works with staff on process improvement solutions.
  • Collaborates with clinic and maintenance staff to maintain the facility and ensure equipment and supplies are in good working order.
  • Performs other duties as assigned.

What You’ll Do:

  • Serves as subject matter expert for professional standard and expectation to all nursing and Medical Assistant staff.

  • Serves as primary point of contact for nursing education and help support facilitation request in a timely manner

  • Consistently monitors and evaluates nursing practices to maintain excellence in standards of care. Support the Regional Director of Nursing to implement quality improvement initiatives in all regions.

  • Develops, implements, and evaluates clinical educational programs using the most effective and best practice recommendations for clinical staff.

  • Coordinates clinical education program and activities, as well as assists in tracking progress of the clinical team using various methods of program evaluation.

  • Collaborates with the Regional Director of Oregon to manage and communicate clinical performance of clinical staff and standard of practice are met as identified and emphasized by the organization.

  • Facilitates in supporting intermittent and ad-hoc clinical education need to the clinical staff based on the request of local nursing leaders.

  • Ensures clinical web page is updated with the most recent nursing communication and information as appropriate.

  • Collects, validates, and helps in managing nursing supervisor dashboard update in Epic to ensure the focused key performance indicators are accurately reflected.

  • Collaborates and works with the business intelligence team to ensure the validity of nursing and clinical data. Provides consulting skills on data collection, database analysis, and reports designed to assess efficiency.

  • Organizes and coordinates clinical education program, clinical skills, and competencies, as well as assists in tracking progress of the clinical team using appropriate learning management system resources.

  • Works closely with the Senior Director of Nursing to formulate a clinical education plan for new hire nurses, medical assistants, new graduate nurses, new charge nurses, and nursing supervisors, ensuring role and expectation discussion.

  • Coordinates and collaborates with organizational department/area nursing supervisor and administrator for understanding clinical needs of each clinic.

  • Provides leadership, guidance, counseling and continuing education to staff per clinical leadership request.

  • Prioritize clinical education to meet the needs of the clinic and project, coordinating with the Senior Director of Nursing.

  • Actively participates with Senior Director of Nursing and local nursing leadership to identify and develop clinical education programs focused on helping coach, mentor, and evaluate performance of clinical staff.

  • Partner with Senior Director of Nursing to implement and enforce clinical practice standards to help nursing supervisors disseminate clinical roles and expectation of each clinical staff in the clinics appropriately.

  • Collaborates with all members of care provision team to share knowledge and plan for optimal implementation of clinical education for all nursing staff.

  • Exercises considerable judgment in applying professional knowledge in solving problems within established policies and practices.

  • Identify and discuss gaps in existing policy and practices with Senior Director of Nursing for process improvement.

  • Serves as an expert and resource to nursing supervisors and clinical staff.

  • Coordinates organizational clinical quality initiatives and process improvement projects and ensures close communication with Senior Director of Nursing.

  • Collaborate with Senior Director of Nursing for identifying and ensuring clinical protocol and policy are updated as per organizational policy.

  • Performs clinical leader responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.

  • Informatics Functions and Responsibilities:

  • Serves as a liaison between the EHR applications team, Informatics team, and Nursing. Collaborates with individuals and teams to remove barriers between clinical staff and the EHR applications team.

  • Utilizes cross-functional knowledge of the organization, EHR, and key workflows in order to provide support and guidance to nursing and the EHR applications team.

  • Partners with the Training Team to improve Nursing understanding, utilization, and efficiency in the EHR system.

  • Serves as a nursing representative at the Clinical Advisory Board (CAB) and EHR Advisory Team (EAT) meetings.

  • Provides clinical review for maintenance processes on nursing workflows.

  • Participates in all EHR-related projects as Subject Matter Expert (SME) and represents Nursing needs.

  • Participates and owns informatics build directed to optimize the EHR for Nursing workflow.

  • Reviews clinical data and datasets needed to implement functionality or resolve system issues.

Qualifications:

  • Required Education: Associate Degree in Nursing

  • Preferred Education: Bachelor of Science in Nursing (BSN). Master of Nursing (MSN)- Education is strongly preferred. Master of Nursing (MSN)- Clinical Informatics is strongly preferred.

  • Three (3) years’ experience in clinical nursing education.

  • Two (2) years’ informatics experience with clinical information system implementation, support, and/or optimization. Completed Clinical Informatics Degree may substitute this requirement.

  • Knowledge of Ambulatory Epic is must for facilitating work as nurse clinical informatics in this role

  • Experience with the design, implementation, and/or evaluation of clinical decision support tools, electronic care plans, or other applications.

  • Ambulatory nursing management experience preferred

  • Active Registered Nurse license for the state of practice (WA and Oregon).
