In our medical clinics, Registered Dietitian Nutritionists work alongside medical and mental health providers to address the nutritional needs of primary care patients.Registered Dietitian Nutritionists at primary care sites counsel patients of all ages on nutrition-related conditions (diabetes, cardiovascular disease, weight management, digestive issues, food allergies) and provide preventative education (Well Child Checks, prenatal counseling, healthy families), addressing general nutrition questions and concerns.

The Primary Care Nutrition Services model utilizes Motivational Interviewing, and nutrition interventions are tailored to meet the individual’s lifestyle and support their health goals in a judgment-free, sensitive, and supportive environment.Our Primary Care RDNs are readily accessible in our clinics for warm handoffs from providers. They are proactive in identifying patients to drop in through reports and registries. Same-day access is available at no cost to any patient who has established care at our clinics.

What We Offer

As a dietitian, you will have the opportunity to:

  • Proactively identify patients to counsel through standard protocols
  • Receive warm handoff referrals from the clinic medical team
  • Provide Medical Nutrition Therapy and preventative education through Motivational Interviewing
  • Document visits appropriately using standard protocols in Electronic Health Records
  • Interact and collaborate with care team
  • Participate in clinic and department meetings
  • Collaborate with internal and external parties
  • Participate in process improvement efforts
  • Communicate effectively with the supervisor, clinic manager, and other relevant parties

Qualifications

  • Education: Master’s Degree in Dietetics from a U.S. regionally accredited college, university, or foreign equivalent for new graduates after January 2024. Dietitians credentialed prior to Jan 2024 are allowed to be grandfathered in with a Bachelor’s Degree. Completion of ACEND® (Accreditation Council for Education in Nutrition and Dietetics) accredited academic coursework in a Didactic Program in Dietetics.
  • Experience:One year’s experience in clinic, primary care, outpatient or community setting preferred.
  • Licenses/Certificates/Registration:Registered Dietitian or registration eligible (must obtain RD credential within 180 days of employment). May require the following certifications based on location and job duties performed.
    • Medical Assistant – Registered (WA WIC Dietitians).
    • Certified Dietitian (if going to bill for Maternity Support Services [MSS]).
    • Licensed Dietitian (if going to bill for Medical Countermeasures ([MCM]).
    • International Board Certified Lactation Consultant (IBCLC).
  • Knowledge/Skills/Abilities Required:Bilingual (English/Spanish) preferred. Must demonstrate the ability to communicate at level 9 on the language proficiency scale to receive bilingual differential pay. Ability to effectively interact with adults, infants, children in a professional manner. Ability to effectively collaborate with medical professionals. Ability to adapt and work in a fast-paced environment. Ability to demonstrate flexibility, initiative, and forward thinking. Strong verbal, written and listening communication skills. General knowledge in culturally appropriate practice. Intermediate proficiency with Microsoft Office and Electronic Health Records.

Essential Functions/Responsibilities/Duties

  • Conducts nutritional screening and assessment for patients/clients to include anthropometric and laboratory data, collection of nutrition history, calculation of nutrition adequacy, and identification of drug-nutrient interactions.
  • Utilizes motivational interview to identify desired goals and outcomes of nutrition care in collaboration with patient/client and health care professionals. Considers the variables that may influence the patient/client’s food related behaviors including culture and social determinants of health.
  • Evaluates and provides evidence based nutrition recommendations to address patient/client medical needs. Understands and evaluates patient/client nutrition status and its relationship to complex disease states including diabetes, cardiovascular disease, chronic kidney disease, gastrointestinal disorders, cancer, food allergies and obesity.
  • Collaborates with physicians and other health care professionals to advocate for and implement optimal nutrition care.
  • Provides preventative services and counseling at individual patient/client visits and group meetings.
  • Serves as preceptor providing nutrition education and guidance to dietetic interns and students. Develops training materials and participates in mentoring of staff.
  • Assists in monitoring compliance to Yakima Valley Farm Workers Clinic (YVFWC) procedures, federal qualified health center quality metrics and/or state and federal WIC requirements.
  • Maintains awareness of community demographics and resources. Connects clients to resources.
  • Designs and implements outreach activities to raise community awareness of program services.
  • Completes documentation in the appropriate electronic medical record to document patient/client interaction.
  • Actively participates in assigned meetings to promote care team collaboration and ongoing learning.
  • Performs other duties as assigned.
  • Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.

Essential Functions/Responsibilities/Duties 

  • Coaches, mentors, and trains direct report staff. Provides continuous performance management and conducts employee performance evaluations. Addresses performance and/or behavior issues, clarifying expectations and providing feedback.
  • Supports staff growth and development. Assesses the educational and experience needs of all levels of staff in collaboration with the individual. Encourage staff to seek educational opportunities by incorporating job-related training in addition to mandatory training.
  • Participates in the recruitment and selection of staff. Provides orientation to outline job requirements and expectations, policies and procedures, and proper use of tools and equipment.
  • Coordinates clinic flow, manages clinic or RD schedule, maintains assigned caseload, and leads in the achievement of clinic goals.
  • Conducts or monitors nutritional screening and assessment for clients to include anthropometric and laboratory data, collection of nutrition history, calculation of nutrition adequacy, and identification of drug nutrient interactions.
  • Identifies or monitors desired goals and outcomes of nutrition care in collaboration with client and health care professionals. Considers the variables that may influence the client’s food related behaviors including culture and social determinants of health. Evaluates and provides evidence based nutrition and breastfeeding recommendations to address client medical needs or monitors such work of RDs. Understands and evaluates client nutrition status and the relationship to complex disease states, including diabetes, gastrointestinal disorders, food allergies, and obesity.
  • Manages and coordinates program outreach events.
  • Provides information about services to health care providers and community partners to improve access to services, promote participation, and provide consistent nutrition and breastfeeding messages.
  • Serves as Preceptor to Dietetic interns and students or mentors those that are Preceptors.
  • Hires, orients and trains staff in compliance with Nutrition Services Procedures and State and Federal requirements. Assists in monitoring staff compliance with procedures, requirements and quality metrics.
  • Assists in the development of Quality Assurance (QA) tools and department procedures to monitor compliance with Nutrition Services Procedures, State and Federal WIC requirements and/or Yakima Valley Farm Workers Clinic (YVFWC) Health and Safety Procedures. Performs QA activities and creates reports according to established procedures. Coaches and mentors staff regarding quality improvement issues.
  • Actively participates in meetings to promote care team collaboration and ongoing learning.
  • Maintains awareness of community demographics and resources. Connects clients and staff to resources.
  • Completes documentation in the appropriate electronic medical record to document client interaction.
  • Performs other duties as assigned.
  • Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.

Qualifications

  • Education: Master’s Degree in Dietetics from a U.S. regionally accredited college, university, or foreign equivalent for new graduates after January 2024.   • Experience: One year’s experience in a WIC or clinical setting is preferred. This includes one year of leadership or supervisory experience preferred. 
  • Professional Licenses/Certificates/Registration: Registered Dietitian or registration eligible required (must obtain RD credential within 180 days of employment). International Board Certified Lactation Consultant (IBCLC) preferred. Valid Driver’s License and proof of liability insurance coverage. May require the following certifications based on location and job duties performed.
    • Medical Assistant – Certified (WA WIC Dietitians).
    • Certified Dietitian (WA Dietitians – if going to bill for MSS).
    • Licensed Dietitian (OR Dietitians – if going to bill for MCM).
    • International Board Certified Lactation Consultant (IBCLC).
  • Knowledge/Skills/Abilities Required: Bilingual (English/Spanish) preferred. Must demonstrate the ability to communicate at level 9 on the language proficiency scale to receive bilingual differential pay. Ability to effectively interact with adults, infants, and children in a professional manner. Must be able to collaborate with medical professionals and have the ability to work in a fast-paced environment.
