New Hope is a nurse-driven clinic at Yakima Medical Dental, a Yakima Valley Farm Workers Clinic. New Hope specializes in caring for patients living with HIV in Central Washington, offering medical, dental, nutrition, counseling, outreach, and mental health services. We are a leader in the Central WA region for HIV care. Public health nurses and case managers collaborate to provide patients with wrap-around care services. What You’ll Do:
Responsibilities for HIV patient care include monitoring health, administering medications, providing emotional support, conducting wellness activities, planning care, educating, administering medications, controlling infection, coordinating services, advocating, managing crises, and continuing education.
Qualifications:
- Associate’s Degree or higher in Nursing
- WA state RN license
- If an RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply: Must submit proof of license in state of residence. Must submit proof of completion for the required suicide prevention training during 1st week of onboarding. If relocating to Washington State, obtain a WA RN license within 6 months of hire. Must update the primary state of residence within 60 days (applies to anyone relocating to Washington State). Military Nurses are not required to obtain a Nurse Licensure Compact (NLC)
- CPR & BLS Certification is required
- Valid Driver’s License and proof of automobile liability insurance coverage
Skills:

As a Registered Nurse, you will have the opportunity to:
- Provide comprehensive care during patient visits, discuss chief complaints, obtain vital signs, complete health history, and document health information in the EMR system
- Conduct phone triage to assess patient needs and coordinate follow-up care
- Assist with diagnostic procedures and treatments, including IV starts when ordered, while educating patients on procedures and medications
- Collaborate with the healthcare team to develop and implement patient care plans, coordinate referrals, and provide education to patients on medications and potential side effects
Qualifications:

As a Clinic Nurse Supervisor, you will have the opportunity to:
- Coach, mentor, and train staff, assessing competencies and providing continuous performance management
- Support staff growth and development by assessing educational and clinical needs, collaborating with individuals on opportunities
- Collaborate with Nursing and Medical Assistant staff on educational initiatives and performance improvement
- Participate in staff recruitment, selection, and orientation, ensuring compliance with policies and procedures
- Develop department work plan, delegate work, and monitor compliance with clinic standards and regulations
- Lead patient care delivery planning, workflow implementation, and process improvement initiatives

Qualifications:

Essential Functions/Responsibilities/Duties
Program Leadership:
- Develop and oversee chronic care case management initiatives, focusing on high-risk patient populations.
- Collaborate with interdisciplinary teams to create patient-centered strategies.
- Monitor program outcomes and adjust as needed to optimize patient care.
Ensure Care Plan Implementation and adherence:
- Develop and train staff on creating/writing care plans and documentation standards.
- Ensure adherence to evidence-based practices and quality standards and advocate for preventive care and early intervention.
Population Health Strategies:
- Analyze data to identify trends and opportunities for improvement.
- Drive initiatives that enhance patient outcomes and reduce healthcare disparities.
Team Leadership:
- Lead and mentor a team of RN care managers and healthcare professionals.
- Foster a collaborative and supportive work environment.
Quality Assurance and Compliance:
- Ensure compliance with regulatory requirements.
- Monitor program quality and adherence to standards.
- Participate in quality improvement efforts.
Qualifications
- Education/Experience:
- Bachelor’s in Nursing and at least 5 years of experience as an RN in care coordination, case/care management, utilization/resource management, and/or transition of care, OR
- Associate’s in Nursing and at least 7 years of experience as an RN in care coordination, case/care management, utilization/resource management, and/or transition of care.
- Of total RN experience, at least 2 years of that must be in a supervisory position.
- Professional Licenses/Certificates/Registration:
- Registered Nurse (RN) with current licensure to practice in Washington State or hold a valid Multi-State RN license.
- If an RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply:
- Must submit proof of license in the state of residence.
- Must submit proof of completion for the required suicide prevention training during 1st week of onboarding.

What We Offer
- $38.53-$47.20/hour DOE with the ability to go higher for highly experienced candidates

As a Registered Nurse, you will have the opportunity to:
- Provide comprehensive care during patient visits, discuss chief complaints, obtain vital signs, complete health history, and document health information in the EMR system
- Conduct phone triage to assess patient needs and coordinate follow-up care
- Assist with diagnostic procedures and treatments, including IV starts when ordered, while educating patients on procedures and medications
- Collaborate with the healthcare team to develop and implement patient care plans, coordinate referrals, and provide education to patients on medications and potential side effects
Qualifications:
- Associate’s Degree or higher in Nursing
- WA State RN license
- If an RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply:Must submit proof of license in state of residence.Must submit proof of completion for the required suicide prevention training during 1st week of onboarding.If relocating to Washington State, must obtain WA RN license within 6 months of hire.Must update the primary state of residence within 60 days (applies to anyone relocating to Washington State).Military Nurses are not required to obtain a Nurse Licensure Compact (NLC)
- BCLS certification within 90 days of hire; ACLS & PALS are preferred

Position Highlights:
- $20.81-$25.50/hour DOE with the ability to go higher for highly experienced candidates
As a Pharmacy Technician, you will have the opportunity to:
- Enters patient data in the system including patient name, insurance, allergies, doctor and diagnosis. Enters the prescription in the system, selecting the brand, product/drug, and appropriate strength to align with the instructions given by the prescribing Provider.
- Fills or dispenses the product from the pharmacy stock and delivers to patient.
- Processes prescription refills verifying refill availability.
- Maintains awareness of stock level and enters order for restocking based on defined minimum inventory levels. Verifies quantity and submits order to the Wholesaler.
- Verifies product orders received with invoice and purchase order. Places a sticker on each item and places on the shelf.
- Monitors and checks expiration dates on drugs in stock and prepares expired stock for return to Wholesaler. Processes credit invoices associated with the returns.
- Returns or discards dispensed prescription to stock if not picked up by patient in 30 days. Enters information in the system for reverse billing.
- Completes third party billing form and contacts Provider if prior authorization is required.
- Performs other duties as assigned.
- Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA) and YVFWC requirements.
Qualifications
- Education:High School Diploma/GED
- Professional Licenses/Certificates/Registration:
- Pharmacy Technician license for states of practice.
- Alternatively, may have an active Pharmacy Intern License for the states of practice.
- Knowledge/Skills/Abilities:Bilingual (English/Spanish) preferred. Candidates must demonstrate the ability to communicate at level 9 on the language proficiency scale to receive bilingual differential pay. Effective verbal and listening communication skills with a customer-oriented approach. Knowledge of medical terminology. Ability to perform administrative tasks such as answering phones, filing, faxing, scanning, and copying. Ability to work as part of a team in a fast-paced environment. Ability to organize work while performing multiple tasks requiring attention to detail. Basic proficiency with a variety of computer systems, including Microsoft Office, is preferred. Basic knowledge of a minimum of one pharmacy software system and ability to learn new systems.
- Drug testing:this job will require a pre-employment drug screen which would take place after extending a conditional offer of employment.

What You’ll Do:
- Coach, mentor, and train staff; conduct performance evaluations and address issues
- Support staff growth by identifying training needs and creating action plans
- Participate in recruitment, conduct orientations, and manage workflow
- Provide patient care, document health history, and assist in procedures and treatments
- Communicate with patients about medications and educate them on care plans
- Collaborate with healthcare providers to develop and implement patient care plans
- Plan meetings, monitor program progress, and review daily processes for improvement
- Communicate with insurance companies and managed care plans as directed
- Ensure clinical responsibilities align with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.
Qualifications:
- Associate’s Degree or higher in Nursing
- One year’s experience as a Registered Nurse (RN) is required; Two years of supervisory experience is preferred

What You’ll Do:
- Provide client care, conduct initial health assessments, and develop care plans based on health history and needs
- Review referrals for Maternal and Pediatric clients, determining needs and collaborating with healthcare providers to develop care plans
- Maintain ongoing communication with clients and professionals to coordinate services and adjust care plans as needed
- Conduct office/home visits for pre/postnatal clients and newborn assessments, providing interventions and education
- Document intervention activities, mentor staff, and students, and ensure compliance with guidelines and requirements
Qualifications:
- Education: Bachelor’s Degree in Nursing; Associate’s Degree with two years experience in parent-child nursing may be substituted for the Bachelor’s degree
- Experience: Six months of experience providing newborn exams is preferred
- Licenses/Certificates/Registration:
- WA State RN License; If RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply:
- Must submit proof of license in state of residence.
- Must submit proof of completion for the required suicide prevention training during 1st week of onboarding.
- If relocating to Washington State, must obtain WA RN license within six months of hire
- Must update primary state of residence within 60 days (applies to anyone relocating to Washington State)
- Military Nurses are not required to obtain a Nurse Licensure Compact (NLC)
- BLS (Basic Life Support) Certification within 90 days of hire

What You’ll Do:

As a Registered Nurse – Service Center Nurse, you will have the opportunity to:
- Patient Triage: Assess the urgency of patient needs, gathering pertinent information to ensure appropriate responses and directing care as necessary.
- Patient Inquiries: Respond to patient questions regarding medications, treatments, and general healthcare information.
- Care Coordination: Guide patients to the right resources, send messages to providers, and advise on next steps. Work with the healthcare team to create and implement care plans, coordinate referrals, and educate patients about medications and side effects.
- Efficient Communication: Collaborate with our local team of nurses to ensure consistency, quality, and warmth in every patient interaction.

What You’ll Do:
- Provide client care, conduct initial health assessments, and develop care plans based on health history and needs
- Review referrals for Maternal and Pediatric clients, determining needs and collaborating with healthcare providers to develop care plans
- Maintain ongoing communication with clients and professionals to coordinate services and adjust care plans as needed
- Conduct office/home visits for pre/postnatal clients and newborn assessments, providing interventions and education
- Document intervention activities, mentor staff, and students, and ensure compliance with guidelines and requirements
Qualifications:
- Bachelor’s Degree in Nursing; Associate’s Degree with two years experience in parent-child nursing may be substituted for the Bachelor’s degree
- Six months of experience providing newborn exams is preferred
- WA State RN License. If RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply:
- Must submit proof of license in state of residence.
- Must submit proof of completion for the required suicide prevention training during 1st week of onboarding.
- If relocating to Washington State, must obtain WA RN license within six months of hire
- Must update primary state of residence within 60 days (applies to anyone relocating to Washington State)
- Military Nurses are not required to obtain a Nurse Licensure Compact (NLC)
- BLS Certification within 90 days of hire
- International Board Certified Lactation Consultant (IBCLC) preferred but not required
- Valid Driver’s License and proof of automobile liability insurance coverage

About Pacific Pediatrics
Pacific Pediatrics, part of the Yakima Valley Farm Workers Clinic network, is dedicated to providing comprehensive healthcare services to children in the Woodburn community. Our clinic offers a range of services, including pediatric care, behavioral health, nutritional guidance, and access to pharmacy services. We pride ourselves on our friendly, bilingual staff and our commitment to accessible, high-quality care for all families.
We are looking for someone who:
- Can communicate in English and Spanish
- Has previous pediatric nursing experience and is comfortable working with pediatric patients (0-21 years old)
- Has previous experience as a lead or supervisor

Position Highlights:
- $44.11-$54.03/hour DOE with the ability to go higher for highly experienced candidates

What You’ll Do:
- Care for the pediatric community in Woodburn and surrounding communities
- Support staff growth by identifying training needs and creating action plans
- Participate in recruitment, conduct orientations, and manage workflow
- Provide patient care, document health history, and assist in procedures and treatments
- Communicate with patients about medications and educate them on care plans
- Collaborate with healthcare providers to develop and implement patient care plans
- Communicate with insurance companies and managed care plans as directed
Qualifications:
- Associate’s degree or higher in Nursing
- One year’s experience as a Registered Nurse is required
- Oregon State RN license.
- Dual licensure in WA & OR is required within 120 days of hire
- If RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply:
- Must submit proof of license in state of residence.
- Must submit proof of completion for the required suicide prevention training during 1st week of onboarding.

What You’ll Do:

Coach, mentor, and train staff. Manage performance and provide feedback.

Support staff growth. Assess educational needs and encourage job-related training.

Recruit and select staff. Provide orientation on job requirements, policies, and tools.

Review maternal and pediatric referrals from Case Management and clinic staff.

Administer screening and assessment tools.

Develop client Care Plans based on health history and screening data.

Maintain therapeutic client relationships, set goals, monitor progress, and revise Care Plans.

Perform prenatal, postpartum, and newborn care interventions and education, including home visits.

Participate in interdisciplinary team case reviews.

Maintain required documentation.

Mentor nursing students, community health workers, and new Public Health Nursing staff.

Review staff charts for policy compliance.

Ensure site productivity and work with staff on improvements.
Qualifications:

Bachelor’s Degree in Nursing with coursework in public health

Two years’ experience in providing maternity and newborn physical exams. This includes at least one years’ supervisory experience.

Current WA state RN license

CPR certification within 45 days of hire

Bilingual (English/Spanish) skills are preferred
